This is the full PDF version, with some chapters OCRed. We will OCR future chapters as soon as we have time. Although we would be extremely grateful for help from users in sorting this out.
I have attached a Word document with the remaining chapters mostly done, they just need some errors correcting and formatting put in like quote boxes and footnotes. So if people could help sort of these out that would be brilliant. If you can help just say in the comments below and say what chapters you are doing so we don't duplicate any work.
I've added a few more chapters. If anyone wants to help out as well if they start at the end and work backwards maybe we could meet in the middle…
